Training is often treated as a routine function. Courses are assigned, attendance is recorded, and completion rates are reported. This can create the impression that training is working.
In practice, many organisations do not fail because training is absent. They fail because training is not properly governed. Requirements are applied inconsistently, mandatory training is delayed or waived, roles change without the framework being updated, and completion is mistaken for competence.

An effective training policy is not just a document.
